- ✪ ✪ ✪ ✪ ✪My wife recently had a fall and because it was unexplained she had to report this to DVA and they suspended her licence . This incident was reported to DVA on 09/09/2015 and a form was sent out for her doctor to complete and I handed this to a member of staff at reception in the surgery . As it is now 22/10/2015 I phoned DVA to ask why it was taking so long and they informed me that they had not received the form from the relevant doctor . I then phoned the surgery and was told that my wife would have to make an appointment with the doctor in order for him to complete the form ! This was the first time she had heard that she would have to see her doctor to enable him to complete the form as he had told her in his surgery that he didn't see any reason why she should not continue to drive .
